The reporting does not have to be exact, as you may have to remember Outreach meeting over the 2009 year and make an estimate of people numbers who attended the meetings and salvations at those meetings. If you do not know the exact date of meeting the month of the meeting is sufficient. Please only include outreach meetings in the survey table  DO NOT INCLUDE PRAYER MEETINGS OR CHAPTER EXECUTIVE MEETINGS in the survey table. Please answer the questions at the start of the survey and return form to the National Office even if you consider your Chapter inactive in 2009 as knowing this information is important for future chapter planning. It is our desire as the National Office to see the Fellowship become even more active in outreach and witness in 2010 than what it was in 2009.
